The following documents reflect all official changes which have been made as of the Addendum 2 release date on May 26, 2000. Please refer to these revised documents for all required contract provisions.
In Section 7, only Form N-2 has been revised from the original RFP. Please submit these updated forms with all completed proposals. All signature forms such as the Addendum Acknowledgement Form should also be submitted with completed proposals.
Upon receipt of Addendum 2, proposers must sign the Acknowledgement Form and
